Extended EC use may be beneficial for some previous smokers by helping them to maintain some of the subjective rewards of smoking, avoid postcessation weight gain, or prevent relapse. However, although EC use is expected to pose few health risks of smoking, some adverse health outcomes of long-term EC use are likely.25,26 Varenicline is not used long term and so has an advantage in this respect. Participants were included if they smoked at least 10 cigarettes per day for at least 5 years, had expired air carbon monoxide (CO) reading of 9 parts per million (ppm) or greater, were age 25 to 45 years lost vape ursa, and were motivated to stop smoking. All pre-ban to post-ban changes in flavor use were statistically significant in paired t-test. Compared with respondents who used e-cigarettes daily, respondents who used e-cigarettes weekly were more likely to decrease their use of menthol and non-TM flavors and were also more likely to quit e-cigarettes (12% versus 3.7%).
But the MRTP pathway falls far short of encouraging the roughly 18 million adults using menthol cigarettes to switch completely to these alternative products if they cannot quit. MRTPs are defined as products that “reduce harm or the risk of tobacco-related disease,” according to section 911 of the Federal Food lost vape centaurus m200, Drug, and Cosmetic Act (FD&C Act). Interestingly, the battery output voltage also seems to have an impact on the cytotoxicity of the aerosol vapours, with e-liquids from a higher battery output voltage showing more toxicity to A549 cells [30].
Federal regulators have targeted flavored vape liquids to rein in underage sales and use. Early results from a national survey of teen tobacco and e-cig use show vaping among teens continued to rise well into 2019. The National Institute on Drug Abuse found 1 in 4 high school seniors had vaped nicotine products within the last month. For 10th graders, the rate was 1 in 5, and for 8th graders, it was 1 in 11.

Then there are other pouches that contain synthetically made nicotine, which is made in the lab and is not derived from tobacco. I don’t know if it’s always apparent from the label whether it’s synthetic or not, but that’s a nuance that we’ve seen in our research. This trend was exacerbated by aggressive marketing, which often targeted younger audiences with social media campaigns and appealing flavors, leading to widespread use among teens throughout the decade.
All this evidence would suggest that e-cigarettes are potentially less harmful than conventional cigarettes (Fig. 2) [11, 14, 22, 24, 27,28,29]. Importantly, however, most of these studies have investigated only short-term effects [10, 14, 15, 22, 27,28,29, 31, 32], and the long-term effects of e-cigarette consumption on human health are still unclear and require further study. The majority of cases reported to poison centers are due to accidental exposures in children less than 6 years old. This reflects the increasing availability of these products in the home due to use by guardians or older siblings. Additionally, in the last 5 years, the FDA has warned companies selling e-cigarette devices made in the shape of toys, foods or cartoons that specifically appeal to use by young people.

As smoking has declined dramatically in the U.S., vaping has gained tremendous popularity very quickly. A 2018 study reported 20% of high schoolers vaped at least once in the past 30 days. They are also touted by some users as a “safer” alternative to smoking, and as a way to either quit smoking cigarettes, or to smoke in places where cigarette smoking is not allowed. In 2021, about 53% of youth reported using disposables, followed by approximately 29% who used refillable or prefilled cartridges. Puff Bar and Vuse are now more popular with youth than Juul, a former e-cigarette market leader. In turn, public health experts grew concerned with youth’s exposure to nicotine.

The increase in e-cigarette use (also called vaping) by kids and young people in recent years is a serious public health threat. Since being introduced to the U.S. market in 2007, e-cigarette use among youth has increased to epidemic levels (Surgeon General’s Advisory on E-cigarette Use Among Youth, 2018). The availability of flavored e-cigarettes is one of the most commonly cited reasons for e-cigarette use among youth (Tsai et al., 2018). Given the importance of flavors contributing to use of e-cigarettes among youth, these data briefs build on this previously published article, providing an update on trends in unit sales of e-cigarettes in the U.S. by product and flavor type. These retail sales data briefs are intended to provide information about population trends in sales for participating retailers; they do not include online sales or vape store sales and cannot be used to make conclusions about subgroup purchasing or behaviors. These estimates are based on the information available at the time of publication and may be subject to updates as more information becomes available.

Pollutants in indoor air, exhaled nitric oxide (NO) and urinary metabolite profiles were analysed. The results of this acute experiment revealed that e-cigarettes are not emission-free, and ultrafine particles formed from propylene glycol (PG) could be detected in the lungs. In line with these observations, dysregulation of lung homeostasis has been documented in non-smokers subjected to acute inhalation of e-cigarette aerosols [10]. While tobacco products have been a long-standing public health issue, e-cigarette (aka e-cigs, vape pens, vapes) use has continued to gain popularity throughout the last decade.
They are often called e-cigs, e-hookahs, vapes, vape pens, tank systems, or mods. They come in many different shapes and sizes—some look like a regular cigarette, some look very different. Approximately 31% of youth adults ages report having used an e-cigarette at least once in their life.
Enter e-cigarettes, which were new, high-tech, and came with no proven health risks. There were no long-term studies yet, but common sense dictated that if you wanted to quit inhaling tobacco through smoking, the least you could do was switch to e-cigarettes. To date, the FDA has authorized only 23 tobacco-flavored e-cigarette products and devices. These are the only vape products legally marketed and sold in the U.S. We do know from my research that they can cause acute adverse events. Some people have reported developing mouth lesions from using them too much.
That’s not to say that e-cigarettes haven’t helped some people transition away from conventional cigarettes. In the meantime, Dr. Baldassarri says population rates of smoking and vaping should be closely monitored to ensure that the regulatory steps taken by the FDA are optimizing protection of public health. “We want to see cigarette smoking rates decline, and we want vaping to decline among youth and non-smokers,” he says. The devices come in various shapes and sizes and are known by different names including “e-cigs” and “vapes.” Using an e-cigarette is sometimes called “vaping,” The e-cigarette is typically reusable with new cartridges or liquid refills depending on the brand and model. E-cigarettes typically operate on a rechargeable lithium-ion battery.
The nicotine content of the liquid can range from “very high” to zero. Refills come in flavors such as cool cucumber, mango, and mint, which may seem natural and harmless, but a single JUUL refill contains as much nicotine as a pack of 20 cigarettes. Some of the ingredients in e-cigarettes could also be harmful to the lungs in the long-term.
“We’re at risk of dealing with nicotine addiction, several health concerns and potential lung diseases that we hadn’t really seen previously or at this rate from a particular product,” Dr. Joshua Mansour, a Los Angeles-based oncologist, told Drugwatch. Teen use of e-cigarettes has increased rapidly since the introduction of Juul in 2015. Surgeon General’s office found about 20 percent of high school students used an e-cigarette. While these devices may help some people quit smoking, there is growing evidence that e-cigarettes can pose serious health risks, especially to people who do not smoke traditional cigarettes. The composition of e-liquids requires stricter regulation, as they can be easily bought online and many incidences of mislabelling have been detected, which can seriously affect consumers’ health. Beyond their unknown long-term effects on human health, the extended list of appealing flavours available seems to attract new “never-smokers”, which is especially worrying among young users. Additionally, there is still a lack of evidence of e-cigarette consumption as a smoking cessation method. Indeed, e-cigarettes containing nicotine may relieve the craving for smoking, but not the conventional cigarette smoking habit.
Adults who report puffing e-cigarettes, or vaping, are significantly more likely to have a heart attack, coronary artery disease and depression compared with those who don’t use them or any tobacco products, according to the study findings. The research lacks a control group of teens who did not vape at all, but the pattern evident within a US sample of 200 e-cigarette users who avoided cigarettes is still concerning. For the sake of public health, the researchers argue for further investigation into the potential toxicity of e-cigarettes. Finally, the researchers compared the levels of nicotine in the blood serum of people after they had vaped e-cigarettes with the levels in people who smoked traditional cigarettes. They found that the amounts of nicotine in the blood were similar between the two groups after 10 minutes of smoking at a constant rate. The flavoring liquid for electronic cigarettes, or e-cigarettes, may increase the risk of cardiovascular disease when inhaled, according to a study led by researchers at the School of Medicine. Public health groups want the FDA to more aggressively police regular cigarettes and flavored e-cigarettes that appeal to teenagers.
The majority of respondents currently smoked cigarettes either daily (30.9%) or weekly (44.4%). Most respondents had been aware of the ban before the survey (67.7%), with a plurality feeling neutral about the ban (36.1%) but more supporting the ban than opposing it (35.9% vs. 28.1%). Among respondents who knew about the ban before the survey, the perceived compliance of local retailers with the ban as lower than 50% was quite high (66.2%), and relatively few perceived that more than 75% of retailers were complying (14.8%). On Dec. 21, 2022, California enacted Senate Bill 793, which prohibited the sale of most flavored tobacco products, including e-cigarettes, to people of all ages. Hookahs, premium cigars and loose-leaf tobacco were exempted from the legislation.
Small and sleek, these products can look a lot like a computer flash drive or pen. This makes it tricky for adults to recognize them right away as vaping tools. Pod-based e-cigarette devices are easy to hide in a fist or a pocket. The CDC’s Office on Smoking and Health (OSH) developed this presentation to educate youth on e-cigarettes, including the health risks, the factors that lead to e-cigarette use, and what youth can do to avoid all tobacco products, including e-cigarettes.
Compared with other race/ethnicity groups, Hispanics were the least likely to quit and Blacks were the most likely to continue using banned flavors after the ban. Respondents were both less likely to quit e-cigarette use and more likely to continue using banned flavors if they had used e-cigarettes for a greater amount of time or had weaker intentions to quit before the ban. Respondents who used e-cigarettes because of the flavor were more likely to continue using banned flavors (statistically significant for non-TM flavors but not for menthol). Those primarily using non-TM flavors before the ban were most likely to quit using e-cigarettes and more likely to continue the same flavor afterward.
The agency has taken a multitude of actions to keep ENDS out of the hands of youth, from policy making to enforcement to education. The age-standardized prevalence of current and daily e-cigarette use was 6.9% (95% CI, 6.7%-7.1%; weighted sample approximately 15 million) and 3.2% (95% CI, 3.1%-3.4%; weighted sample approximately 7 million), respectively (Table 2). Among individuals who reported current e-cigarette use, the proportion of daily use, as a measure of established use and possible nicotine addiction, was 46.6% (95% CI, 45.3%-48.0%) (Table 2 and eTable 1 in Supplement 1). Many themes in e-cigarette marketing, including sexual content and customer satisfaction, are parallel to themes and techniques that the tobacco industry aimed at youth and young adults in their advertising and promotion of conventional cigarettes. More than 6 of 10 youth believe that occasional use of e-cigarettes causes only little or some harm. The Sonoma County Board of Supervisors today voted to ban the sale of flavored tobacco products and e-cigarettes in unincorporated areas of the county. The changes to the county’s tobacco retail ordinance are in response to public health data that show e-cigarettes and flavored tobacco use increasing among high school-aged youth. Yet, they contain many substances beyond nicotine, including propylene glycol, glycerin, flavorings and potentially harmful chemicals such as formaldehyde and metals, which could pose significant health risks such as respiratory disease, cardiovascular disease and cancer. Vaping also is strongly linked with a serious medical condition that damages the lungs due to the vitamin E acetate, an additive used in tetrahydrocannabinol-containing e-cigarettes.

In one study, our team looked at the marketing claims used by two FDA-authorized MRTPs, and found that only 1 in 4 people who smoke cigarettes believe that a product with MRTP claims is much less harmful or addictive than cigarettes. The Department has sent a statewide health alert to health care providers and local health departments following recent reports of severe lung disease in people who have used vaping products. The cytotoxic and pro-inflammatory effects of different e-cigarette flavouring chemicals were also tested on two human monocytic cell lines—mono mac 6 (MM6) and U937 [86]. This is the first study to report human AM responses to ECVC and demonstrates dose-dependent cytotoxicity, inducing apoptosis with both nicotine dependent and independent responses which the vaping process accentuates. At sub-cytotoxic doses, ECVC enhances production of ROS, inflammatory cytokines, chemokines and metalloproteinases, although the response is less pronounced with nfECVC. Bacterial phagocytosis by macrophages is inhibited acutely by ECVC and the effects are attenuated by the anti-oxidant NAC, suggesting ROS and reactive aldehydes play a role in the effects of ECVC/nfECVC. ROS production in response to cigarette smoking (or smoke extract) has been implicated as a mediator of adverse effects,23 therefore we examined the possible utility of N-acetyl cysteine (NAC) treatment in reducing the harmful effects of ECVC in THP-1 macrophages. Both AM and THP-1 macrophages were used for these experiments due to the large number of experimental conditions and numbers of cells required.
For example, tobacco-flavored e-cigarette cartridges sales dropped from 47.2% of those sold or given away in 2015 to 21% in 2018, while the sale of “other” flavored cartridges tripled in that time, increasing from 13.8% in 2015 to 42.1% in 2018. This was significant because Chantix, at one point, had a “black box warning” after reports linking the drug to psychiatric side effects. That warning was dropped in 2016 after a very large study showed the drug to be safe, but Toll and Fucito said the stigma of the warning remains in the minds of both health care providers and the general public. Electronic cigarettes (e-cigarettes) and other vapor products are devices used to heat and aerosolize (turn into particles) highly addictive nicotine or other substances that a person can inhale into their lungs. Nonetheless, the 2021 NYTS provides crucial information about youth use of e-cigarettes. Vaping is when you use a handheld electronic device to breathe a mist (“vapor”) into your lungs.
Youth and young adults had more than three times the odds of using fruit-flavored e-cigarettes compared to older adults. A 2017 study by Truth Initiative found 25% of survey respondents, aged 15-24, recognized a JUUL e-cigarette device when shown a photo of the product. And among those who recognized JUUL, 25% reported that use of this product is called “JUULing,” indicating that this product is so distinctive, it’s perceived as its own category. However, it’s likely that recognition is higher now given that JUUL’s sales market share continued to rise throughout 2018 and top out at 75% among U.S. e-cigarette sales in July 2019. With Americans puffing less each year, the cigarette industry sees ESDs and other smokeless products as financial saviors.

E-cigarettes, aka JUULs and vape pens, use a battery to heat up a special liquid into an aerosol that users inhale. The “e-juice” that fills the cartridges usually contains nicotine (which is extracted from tobacco), propylene glycol, flavorings and other chemicals. Studies have found that even e-cigarettes claiming to be nicotine-free contain trace amounts of nicotine. Additionally, when the e-liquid heats up, more toxic chemicals are formed. In 2016, the FDA established a rule for e-cigarettes and their liquid solutions.
These products are also sometimes called “JUULs” (after a branded e-cigarette of the same name), “vapes,” and “vape pens.” Governor Gretchen Whitmer has taken aggressive action to protect Michigan kids from the harmful effects of vaping, including making Michigan the first state in the nation to ban flavored nicotine vaping products. To protect kids now, states and cities must also continue their growing efforts to end the sale of flavored e-cigarettes, as well as other flavored tobacco products.

Principal investigator Wendy Max, PhD, director of the Institute for Health & Aging, noted that from 2013 to 2018, e-cigarette use among high school students soared from 4.5% to 20.8%. Protective factors may reduce the risk of youth engaging in substance use. Increasing protective factors in addition to reducing risk factors can be more effective in preventing substance use. Another piece of good news – those who stopped vaping didn’t boomerang back to cigarettes. None of the participants in this study experienced serious side effects, although a larger study would be needed to verify this finding.
Participants took one vape from an e-cigarette, with at least 12 mg/mL of nicotine lost vape disposable, or inhaled a conventional cigarette, every 20 s for 10 min. Blood samples were collected 1, 2, 4, 6, 8, 10, 12 and 15 min after the first puff, and nicotine serum levels were measured by liquid chromatography-mass spectrometry (LC–MS). The results revealed higher serum levels of nicotine in the conventional CS group than in the e-cigarette group (25.9 ± 16.7 ng/mL vs. 11.5 ± 9.8 ng/mL).
You may have heard the benefits of vaping are that it’s safe or that it helps you quit smoking. Some people vape because they think it’ll help them quit smoking if they’ve already tried other methods and haven’t been able to quit. Unfortunately, you may end up addicted to vaping instead, so the benefits of vaping for smoking cessation may not be as good as they seem. Vaping is when you use a small, handheld device (like e-cigarettes, vape pens or mods) to inhale a mist of nicotine and flavoring (e-liquid). It’s similar to smoking a cigarette, but vaping heats tiny particles out of a liquid rather than burning tobacco. E-cigarettes as consumer products have not been proven to be effective for cessation at the population level.
This may, at least in part, be due to ingredients of vape products, including vitamin E acetate, flavorings, and other additives. Flavors and other ingredients may combine to form harmful chemicals when heated. In addition to vitamin E acetate, several non-marijuana-based ingredients you inhale can harm your lungs. Some experts say chest X-rays of people with EVALI show signs of lung irritation by oily chemicals, which could include both vitamin E acetate and THC oil itself. Marijuana vaporizers are typically pen-like devices with an opening to inhale the vapor from tetrahydrocannabinol (THC) oils, or to use marijuana concentrates made from parts of the cannabis plant that contain high THC levels.
Unlike traditional cigarettes, e-cigarettes are made up of a battery-operated heating part — a cartridge (unit) that typically holds nicotine and other chemicals that change into a chemical-filled aerosol when heated. As a result of the FDA’s missed deadlines and inadequate enforcement, flavored e-cigarettes remain widely available online and in stores across the country. Every day flavored e-cigarettes remain on the market, our kids remain at risk. The FDA must act without further delay to remove all flavored e-cigarettes from the market. Despite not authorizing a single synthetic nicotine product, it has allowed these products – including e-cigarettes in kid-friendly flavors – to stay on the market well past the July 13 deadline.
