This study replicated the previous findings of ECs being more effective than NRT despite only 3% of participants having had tried treatment with NRT before. There is no good evidence that second-hand vapour from e-cigarettes is harmful. As vaping is still relatively new, we can’t be sure there aren’t any long-term effects to people who breathe in someone else’s vapour. E-cigarettes should only be used to help you stop smoking, or to stop you going back to tobacco.

Reported sales of cartridge products increased from $2.133 billion in 2020 to $2.496 billion in 2021; sales of disposable, non-refillable e-cigarette products increased from $261.9 million in 2020 to $267.1 million in 2021. The market share of disposable e-cigarette products increased from 52.0% to 57.8% between 2022 and 2023. Among disposable e-cigarette products, those with flavors other than tobacco accounted for 94.7% of the market in 2023.

In December 2019, the legislation was included in the federal year-end legislative package and passed by both houses of Congress. President Trump signed the bill into law on December 20, 2019 and it immediately took effect. This legislation, which applies to all states, raises the minimum age of sale for all tobacco products— including e-cigarette—to 21. Numerous in vivo and in vitro studies have been performed to better understand the impact of these new inhalable compounds on human health. Results of toxicological analyses suggest that e-cigarettes can be safer than conventional cigarettes, although harmful effects from short-term e-cigarette use have been described. Worryingly, the potential long-term effects of e-cigarette consumption have been scarcely investigated. In this review, we take stock of the main findings in this field and their consequences for human health including coronavirus disease 2019 (COVID-19). The prevalence of e-cigarette use by adults in the United States continues to rise.

The evidence for e-cigarettes as a tool to help adults quit smoking is limited. To date, no e-cigarettes have been approved by the FDA as smoking-cessation devices. In 2019, more than one in six (15.2%) New York City high school students reported using e-cigarettes.

For one, our protocol for the study was developed during the COVID-19 pandemic. With the health and safety of our team in mind, we avoided in-person interactions. This means that we did not receive the package of flavored e-cigarettes from a delivery person. A comprehensive evaluation of SB 793 compliance among brands and vendors that sell their products online in California would help determine the extent to which flavored e-cigarettes are still available. This research would provide data on retailer awareness of the new legislation and would show whether they understand the potential consequences for being in violation of the new law. Interestingly, most of these reports linking COVID-19 harmful effects with smoking or vaping, are based on their capability of increasing the expression of angiotensin-converting enzyme 2 (ACE2) in the lung. It is well known that ACE2 is the gate for SARS-CoV-2 entrance to the airways [106] and it is mainly expressed in type 2 alveolar epithelial cells and alveolar macrophages [107]. To date, most of the studies in this field indicate that current smokers have higher expression of ACE2 in the airways (reviewed by [108]) than healthy non-smokers [109, 110]. The range of e-liquid flavours available to consumers is extensive and is used to attract both current smokers and new e-cigarette users, which is a growing public health concern [6]. In fact, over 5 million middle- and high-school students were current users of e-cigarettes in 2019 [75], and appealing flavours have been identified as the primary reason for e-cigarette consumption in 81% of young users [76].

The 2019 data revealed that 45% borrowed or “bummed” them, 14% gave money to other people to buy them and 8% were given the products by adults. My reading of the evidence is that smokers who switch to vaping remove almost all the risks smoking poses to their health. Smokers differ in their needs and I would advise them not to give up on e-cigarettes if they do not like the first one they try.

Therefore, follow up studies are necessary to clarify the impact of e-cigarette consumption on human health in SARS-CoV-2 infection. Recently, a commonly commercialized crème brûlée-flavoured aerosol was found to contain high concentrations of benzoic acid (86.9 μg/puff), a well-established respiratory irritant [88]. When human lung epithelial cells (BEAS-2B and H292) were exposed to this aerosol for 1 h, a marked cytotoxicity was observed in BEAS-2B but not in H292 cells, 24 h later.

The Federal Trade Commission’s first-ever report on e-cigarette products paints a disturbing picture of surging e-cigarette sales and advertising that are likely to damage the health of America’s youth. “People can get to very high levels of nicotine exposure with these e-cigarette products, and they can use them near constantly throughout the day. So, the question we all have is, ‘Can any pharmacotherapy stand up to this challenge? Talk to your patients, including youth and young adults, about the dangers of tobacco use. Screen all patients, encourage them to quit, and refer patients to evidence-based services for help quitting.

The American Academy of Pediatrics and the American Lung Association are glad to have the rules. But, Sward adds, “We certainly thought [the FDA] should have gone farther — ending the sale of flavored products.” Some come in candy and fruity flavors that appeal to kids and teens. “Diacetyl is a well-known harmful chemical, which, among other things, causes a lung disease called ‘popcorn lung,'” says Erika Sward, assistant vice president for national advocacy at the American Lung Association. Sonoma County’s ordinance, which will take effect 30 days after passing, aligns with the cities of Petaluma, Sebastopol and Windsor, which have also outlawed the sale of e-cigarettes and flavored tobacco.

“EVP use is not a safer alternative to smoking but may have contributed to the decline in regular tobacco product use. EVP use also raises concerns about new health risks, including nicotine addiction.” Electronic vapor products (EVPs), also known as e-cigarettes or vaping devices, have an allure because of their marketed image as a safer alternative to traditional cigarette smoking and for their variety of appealing flavors. Preventing youth use of e-cigarettes and other vaping devices will require community-based efforts to monitor the activities of youth and decrease youth access to devices, e-liquids, and other illicit substances.

Then there’s tobacco-flavored e-cigarettes, and in that case the benefit–harm equation is different, because tobacco-flavored e-cigarettes are not nearly as popular with kids as flavored e-cigarettes. And so that’s how we have been able to authorize some tobacco-flavored e-cigarettes for marketing. This is because under federal law, although the FDA has the power to regulate the tobacco industry, they can do so with certain limitations. For example, nicotine and cigarette products cannot be banned, but flavorings such as mint (sold as menthol) and fruit can.

Analyses from the Population Assessment for Tobacco and Health study show that the leading reason for youth and young adult e-cigarette use is “they come in flavors I like” — with 77.9% and 90.3% selecting this as a reason, respectively. In comparison, only 66.4% of adults aged 25 and over selected this as a reason. Preliminary data published in Nicotine & Tobacco Research showed that after just two weeks of using This is Quitting, more than half of participants — 60.8% — reported that they had reduced or stopped using e-cigarettes. Protective factors may reduce the risk of youth engaging in substance use. Increasing protective factors in addition to reducing risk factors can be more effective in preventing substance use. Another piece of good news – those who stopped vaping didn’t boomerang back to cigarettes. None of the participants in this study experienced serious side effects, although a larger study would be needed to verify this finding.

The study led by Dr. Vindhyal included data from a total of 96,467 respondents from the National Health Interview Survey, a Centers for Disease Control and Prevention-fielded survey of Americans, from 2014, 2016 and 2017. In their analyses, researchers looked at the rates of high blood pressure, heart attack, stroke, coronary artery disease, diabetes and depression/anxiety among those who reported using e-cigarettes (either some days or daily) and nonusers. Those who reported using e-cigarettes were younger than nonusers (33 years of age on average vs. 40.4 years old), according to the ACC release. Then there are products that were on the market already and that were the subject of an application that was submitted by the deadline, but we haven’t gotten to their application yet. Although evidence is lacking, e-cigarettes for inhalation of nicotine may be beneficial in reducing adverse health effects related to the use of combustible tobacco products. However, e-cigarettes are not FDA approved as a smoking cessation tool and the amount of nicotine and other substances a person inhales from each nicotine cartridge remains unclear. When FDA “deemed” e-cigarettes as part of its jurisdiction in 2016, it gave e-cigarettes that were currently on the market two years to prepare premarket applications (known as PMTAs). These applications are what FDA uses to to determine whether new tobacco products are “appropriate for the protection of public health” before they are allowed on the market.

Almost half (49.1%) of young people don’t know what to do with used e-cigarette pods and disposable devices. Daily use of EVPs increased from 2% in 2015 to 7.2% in 2019, greater than three-and-one-half times increase. After further lawsuits and delays, some caused by the COVID pandemic, a September 2020 court-ordered deadline was established for companies to submit applications to FDA seeking to market their e-cigarette products in the United States. In this interview, Mitch Zeller, J.D., director of FDA’s Center for Tobacco Products, provides insights into how e-cigarettes are regulated and recent FDA actions on e-cigarettes.

Vaping hasn’t been around long enough to know what kind of long-term damage it might cause. Vaping works by heating liquid in a small device so you can breathe it into your lungs. The e-cigarette, vape pen or other vaping device heats the liquid in the device to create an aerosol. You breathe these particles into your mouth from the mouthpiece, where they go down your throat and into your lungs. To truly help tobacco users quit and to strengthen global tobacco control, governments need to scale up policies and interventions that we know work.

A study in the Journal of the American Medical Association in 2015 found a connection too. Researchers surveyed 2,500 Los Angeles high school students who had never smoked. They found that kids who used e-cigs were more likely than non-users to smoke cigarettes or other tobacco products over the next year.

Participants received a 12-week supply of varenicline (Chantix; Pfizer) and a leaflet with product use instructions. Participants were instructed to start using varenicline from the next day and stop smoking completely from their TQD onward. Participants were then randomized into 1 of the 3 interventions and given their study product and instructions on how to use it.

Using e-cigarettes, or “vaping,” are terms used synonymously to refer to the use of a wide variety of electronic, battery-operated devices that aerosolize, but do not burn, liquids to release nicotine and other substances. Nicotine-containing e-cigarettes are regulated as “tobacco products” by the FDA because the nicotine is derived from the tobacco plant. E-cigarettes pose a threat to the health of users and the harms are becoming increasingly apparent. In the past few years, the use of these products has increased at an alarming rate among young people in significant part because the newest, re-engineered generation of e-cigarettes more effectively delivers large amounts of nicotine to the brain. Many e-cigarettes sold in the U.S. contain far more nicotine than e-cigarettes sold elsewhere, which increases the risk of addiction and harm to the developing brains of youth and young adults. Marketing tactics targeting young people have contributed to the rapid increase in use.

It should be noted that there are seven FDA-approved quit aids that are proven safe and can be effective when used as directed. To date, no e-cigarette has been approved as a cessation device or authorized to make a modified risk claim, and more research is needed to understand the potential risks and benefits these products may offer adults who use tobacco products. The mid-to-long-term consequences of e-cigarettes are not yet known, as it’s a new product and has been sold for less than a decade in the U.S. While much remains to be determined about these lasting health consequences of these products, we are very troubled by what we see so far.
