Dr. Panagiota “Yiota” Kitsantas, another researcher involved in the study, emphasized that nearly all e-cigarettes in the U.S. contain nicotine, posing significant addiction risks. “EVP use increases risks of nicotine addiction, drug-seeking behavior, mood disorders, and long-term risks of avoidable premature morbidities and mortality,” he explained. Marketing tactics targeting young people have contributed to the rapid increase in use.

The difference in opinion worldwide may be due to different restrictions imposed. For example, while no more than 20 ng/mL of nicotine is allowed in the EU, e-liquids with 59 mg/dL are currently available in the United States. Nevertheless, despite the national restrictions, users can easily access foreign or even counterfeit products online. Electronic nicotine dispensing systems (ENDS), commonly known as electronic cigarettes or e-cigarettes, have been popularly considered a less harmful alternative to conventional cigarette smoking since they first appeared on the market more than a decade ago. E-cigarettes are electronic devices, essentially consisting of a cartridge, filled with an e-liquid, a heating element/atomiser necessary to heat the e-liquid to create a vapour that can be inhaled through a mouthpiece, and a rechargeable battery (Fig. 1) [1, 2]. Both the electronic devices and the different e-liquids are easily available in shops or online stores.

Vapes are often viewed and marketed as a safe alternative to smoking. A 2015 study in the Journal of the American Medical Association found that teens who took up vaping were three times more likely to start smoking within six months compared to teens who didn’t vape. A Yale University study in 2018 found the switch from vaping to tobacco cigarettes could happen in as little as one month.

You may have seen ads or stories on the internet that say e-cigarettes are not harmful, or are a good way to help smokers quit smoking. However, doctors and researchers still have a lot to learn about the health effects of e-cigarettes. While e-cigarettes may be less harmful than regular cigarettes, this does not mean that they are harmless. E-cigarettes, devices that typically deliver nicotine, flavorings, and other additives to users through an inhaled aerosol, are a rapidly emerging trend, and are especially popular among youth and young adults.

Respondents who perceived a lower level of local retailers’ compliance to the ban were more likely to obtain e-cigarettes from illegal sellers and mixed flavored e-liquids on their own, compared with those who perceived a higher level of local retailer compliance. A possible explanation is the neighborhood effect, that is, in neighborhoods where law enforcement is relatively weak, local retailers are less likely to follow the ban and illegal sellers are more active.

In addition to its toxicological effects on foetus development, nicotine can disrupt brain development in adolescents and young adults [44,45,46]. Several studies have also suggested that nicotine is potentially carcinogenic (reviewed in [41]), but more work is needed to prove its carcinogenicity independently of the combustion products of tobacco [47]. In this latter regard, no differences were encountered in the frequency of tumour appearance in rats subjected to long-term (2 years) inhalation of nicotine when compared with control rats [48]. Despite the lack of carcinogenicity evidence, it has been reported that nicotine promotes tumour cell survival by decreasing apoptosis and increasing proliferation [49], indicating that it may work as a “tumour enhancer”. In a very recent study, chronic administration of nicotine to mice (1 mg/kg every 3 days for a 60-day period) enhanced brain metastasis by skewing the polarity of M2 microglia, which increases metastatic tumour growth [50].

Vitamin E acetate was present in bronchoalveolar-lavage (BAL) fluid in 48 out of 51 EVALI patients from 16 states, but it was not found in BAL fluid from the comparison group of 99 healthy individuals. E-cigarettes are threatening to addict a new generation to nicotine. E-cigarette use among young people, many of whom were not smokers in the first place, has skyrocketed in recent years. Nearly 20% of high school students, one in five, use e-cigarettes, according to the National Youth Tobacco Survey. Young people who use e-cigarettes are also more likely to start smoking cigarettes than their peers who do not vape. The FDA has also reported that, among current youth users of e-cigarettes, 97% used a flavored e-cigarette in the past month.

The report also found more than half of Juul users were younger than 18 the first time they tried the device. E-cigarettes are available in flavors like cherry, bubble gum, and cotton candy, and are allowed to be advertised on TV flum float flavors, which may increase their appeal for children and teens. E-cigarettes are considered tobacco products because most of them contain nicotine, which comes from tobacco. And more than a quarter (27.6%) of current youth e-cigarette users say they use an e-cigarette product every day. These electronic devices, also known as e-cigarettes, vapes, vape pens, personal vaporizers, e-cigars, pod systems, mods and e-hookah, are not a safe alternative to cigarette smoking. The report, published in the Morbidity and Mortality Weekly Report, was based on data from the 2021 National Youth Tobacco Survey (NYTS), a cross-sectional, self-administered survey of U.S. middle (grades 6–8) and high (grades 9–12) school students. The study assessed current (used on one or more of the past 30 days) e-cigarette use; frequency of use; and use by device type, flavors and usual brand. Centers for Disease Control and Prevention (CDC) estimated that more than 2 million U.S. middle and high school students reported currently using e-cigarettes in 2021, with more than 8 in 10 of those youth using flavored e-cigarettes. In some instances, because of its appearance and the vapours it emits, an e-cigarette may be confused with a regular cigarette. Other types of e-cigarettes, however, are similar to a cigar, a smoking pipe, or a ballpoint pen in appearance.

The FDA reports an alarming 900% rise in e-cigarette use among high school students from 2011 to 2015 and the number of high school students using them increased 78% in 2018 alone. Some individuals who smoke combustible tobacco products, such as cigarettes, cigars, and pipes coil ignite v50, have been turning to electronic cigarettes (e-cigarettes) as an alternative to combustibles and as a smoking cessation tool. Neither the air‐exposed rats nor those exposed to e-cigarette vapor using SS heating elements developed respiratory distress. In contrast, 80% of the rats exposed to e-cigarette vapor using NC heating units developed clinical acute respiratory distress when a 70‐W power setting was employed. Thus, suggesting that operating units at higher than recommended settings can cause adverse effects.

They produce a vapor that’s inhaled deep into the lungs, mimicking the feeling of smoking tobacco cigarettes. E-cigarettes hit the market in the early 2000s and have since soared in popularity, especially among teens and young adults. Once believed to be a “safer” way to smoke, vaping is now considered a public health crisis by many health groups. In Sonoma County, 13 percent of 11th graders said they have used an e-cigarette, according to the Department of Health Services.

However, e-cigarettes containing 20 mg/mL of nicotine are more equivalent to normal cigarettes, based on the delivery of approximately 1 mg of nicotine every 5 min [40]. ESD aerosol is a new source of pollution and toxins being emitted into the environment. We do not know the long-term health effects of ESD use and although the industry marketing of the product implies that these products are harmless, the aerosol that ESD emit is not purely water vapor. In conclusion, we sought to replicate the potential effects of exposure of the user in an acute in vitro system using our vaping-condensate technique.

The age-standardized prevalence of current and daily e-cigarette use was 6.9% (95% CI, 6.7%-7.1%; weighted sample approximately 15 million) and 3.2% (95% CI, 3.1%-3.4%; weighted sample approximately 7 million), respectively (Table 2). Among individuals who reported current e-cigarette use, the proportion of daily use, as a measure of established use and possible nicotine addiction, was 46.6% (95% CI, 45.3%-48.0%) (Table 2 and eTable 1 in Supplement 1). Many themes in e-cigarette marketing, including sexual content and customer satisfaction, are parallel to themes and techniques that the tobacco industry aimed at youth and young adults in their advertising and promotion of conventional cigarettes. The inhalation of harmful chemicals can cause irreversible lung damage and lung diseases. This study examined the impact on adult e-cigarette users from the recent e-cigarette-only flavor bans in Washington State, New Jersey, and New York. We found that most respondents continued to use e-cigarettes with banned flavors post-ban and obtain banned-flavor e-cigarettes through legal channels. Enforcing compliance of retailers to the policy is crucial to control e-cigarette use. First, we depicted the characteristics of respondents from the three states with chi-square tests to evaluate demographic differences by state and the distribution of respondents by their tobacco and nicotine use status.

The majority of respondents currently smoked cigarettes either daily (30.9%) or weekly (44.4%). Most respondents had been aware of the ban before the survey (67.7%), with a plurality feeling neutral about the ban (36.1%) but more supporting the ban than opposing it (35.9% vs. 28.1%). Among respondents who knew about the ban before the survey, the perceived compliance of local retailers with the ban as lower than 50% was quite high (66.2%), and relatively few perceived that more than 75% of retailers were complying (14.8%). On Dec. 21, 2022, California enacted Senate Bill 793, which prohibited the sale of most flavored tobacco products, including e-cigarettes, to people of all ages. Hookahs, premium cigars and loose-leaf tobacco were exempted from the legislation.

A 2021 review found people who used e-cigarettes to quit smoking, as well as having expert face-to-face support, can be up to twice as likely to succeed as people who used other nicotine replacement products, such as patches or gum. Additionally, high quality epidemiology studies consistently demonstrate that e-cigarettes use increases conventional cigarette uptake, particularly among non-smoking youth, by nearly 3 times. Evidence reveals that these products are harmful to health and are not safe. However, it is too early to provide a clear answer on the long-term impact of using them or being exposed to them.

These products are also sometimes called “JUULs” (after a branded e-cigarette of the same name), “vapes,” and “vape pens.” Governor Gretchen Whitmer has taken aggressive action to protect Michigan kids from the harmful effects of vaping, including making Michigan the first state in the nation to ban flavored nicotine vaping products. To protect kids now, states and cities must also continue their growing efforts to end the sale of flavored e-cigarettes, as well as other flavored tobacco products.

Over the years, Evans tried to quit by using the nicotine patch, gum, and even hypnosis. A 2015 expert review from Public Health England estimated e-cigs are 95% less harmful than the real thing. When you stop using it, you can go into withdrawal and feel depressed and crabby. This study was the first to use endothelial cells derived from iPS cells to directly investigate the effect of e-liquids with and without nicotine on their viability and function.

In addition, young adults were significantly more likely to use both cigarettes and e-cigarettes than adults 45 and older. Young adults with family incomes of less than 200% of the federal poverty level also were more likely to use e-cigarettes than those with greater family income. Among young adults, men were slightly more likely to use e-cigarettes than women, while those who are white were more likely to use e-cigarettes than young Black adults and significantly more likely than young Asian or Hispanic/Latino adults. The emergence of vaping-related illnesses, which have prompted federal health agencies’ investigations and advisories, underscore the urgency of research. PHE’s remit letter for 2014 to 2015 requested an update of the evidence around e-cigarettes. PHE commissioned Professors Ann McNeill and Peter Hajek to review the available evidence. The review builds on previous evidence summaries published by PHE in 2014. All of our local NHS Stop Smoking Services now proactively welcome anyone who wants to use these devices as part of their quit attempt and increase their chance of success. Continuous and rigorous implementation of traditional tobacco control measures, such as taxation, clean indoor air policies and public education is also essential. Finally, the recent news of vaping-related illnesses requires swift research and effective guidance to protect the public’s health. In addition to the unknown health effects, early evidence suggests that vaping might serve as an introductory product for preteens and teens who then go on to use other nicotine products, including cigarettes, which are known to cause disease and premature death.

For example, tobacco-flavored e-cigarette cartridges sales dropped from 47.2% of those sold or given away in 2015 to 21% in 2018, while the sale of “other” flavored cartridges tripled in that time, increasing from 13.8% in 2015 to 42.1% in 2018. This was significant because Chantix, at one point, had a “black box warning” after reports linking the drug to psychiatric side effects. That warning was dropped in 2016 after a very large study showed the drug to be safe, but Toll and Fucito said the stigma of the warning remains in the minds of both health care providers and the general public. The American Association of Poison Control Centers, reports as of May 31, 2022, poison control centers have managed 2,451 exposure cases about e-cigarette devices and liquid nicotne in 2022. A study in the Journal of the American Medical Association in 2015 found a connection too. Researchers surveyed 2,500 Los Angeles high school students who had never smoked. They found that kids who used e-cigs were more likely than non-users to smoke cigarettes or other tobacco products over the next year.

We believe this is an important step in establishing an in vitro system by which to investigate the effects of e-cigarettes on the airways. Finally, we have not assessed the effects of flavours on cellular effects,43 partly because this adds another layer of uncertainty and potential confounders. However, the data provide a background on which to study these other potential factors, with and without nicotine.

Like many states, New Jersey made it illegal to sell vaping products to anyone under 21 and then went a step further by banning flavored vape juice. But the laws did little to curb the problem, experts told the panel. Most teens report using vape devices for nicotine, or flavored products only. However, the DEA urges teens (and the adults who care about them) to understand the potential health consequences. Both smoking and vaping marijuana generally have been considered safer than smoking cigarettes (or vaping with nicotine products, which may introduce new health hazards rather than reducing all harm). E-cigarettes are battery-operated devices used for a type of smoking called vaping.

Children 13–15-years old are using e-cigarettes at rates higher than adults in all WHO regions. In Canada, the rates of e-cigarette use among 16–19-year-olds has doubled between 2017–2022, and in England (the United Kingdom) the number of young users has tripled in the past three years. There’s no evidence so far that vaping causes harm to other people around you. Electronic smoking devices (ESDs) do not just emit “harmless water vapor.” Secondhand aerosol (incorrectly called vapor by the industry) from ESDs contains nicotine, ultrafine particles and low levels of toxins that are known to cause cancer. These results underscore the rationale for the implementation and enforcement of public health policies tailored to young adults. Use of electronic cigarettes (e-cigarettes) impairs indoor air quality and increases FeNO levels of e-cigarette consumers. E-cigarette manufacturers are using marketing tactics used by the tobacco industry to lure kids to conventional cigarettes.
